Problem and solution list example :problems could be electrolyte imbalance ,urosepsis calculi etc .solution would be specific medicines prescribed ,dialysis etc.again all cases but a lot of them are similar ..

A. ELECTROLYTE IMBALANCE : 

patient with chronic renal failure ,since there is an absence of renal regulatory mechanisms . in renal failure acute or chronic one most commonly sees patient who have tendency to develop hypervolemia , hyperkalemia ,hypercalcemia, bicarbonate deficiency {metabolic acidosis} electrolyte imbalance after urinary diversion vary depending on this site of urine diversion 

TREATMENT 

intravenous fluids ,electrolyte repalcement . 

a minor electrolyte imbalance may be corrected by diet change for example eating rich in potassium 


MOST COMMONLY CAUSES AN ELECTROLYTE IMBALANCE ARE : 

vomiting 

diarrhea

not taking enough fluids 

excessive sweating

certain medications such as laxatives and diuretics 

eating disorders

liver or kidney disorders cancer treatments

congestive heart failures 

SYMTOMPS OF UROSEPSIS INCLUDE : 

Pain near kidneys ,on the lower sides of the back 

nausea with or without vomiting 

reduced urine volume or no urine

trouble breathing or rapid breathing 

confusion or brain fog

unusual anxiety levels 

changes in heart rate such as pal pitations or rapid heartrate 

weak pulse 

high fever or low body temperature

profuse sweating

A doctor may diagnose urosepsis after confirmimg that the person has a UTI which is done through a simple urine sample if a UTI has been left untreted or doctor thinks the infection may be spread they may order immediate blood tests to help diagnose urosepsis 

if caught early to treat with antibiotics .a person with UTI also needs to drinks plenty of fluids to help flush the urinary tract   , may not respond to antibiotics alone some people will need surgery to get rid of the source of an untreated infection completely

this assigment has helped me to learn the basics of clinical practice like history taking ,presentation of the case and how to approach a patient etc but to be honest when compared to online cls ,offline classes will be more effective and productive .
